Tragic. I can't find much detail, but I remember incidents like this going back more than forty years with a variety of military
sidearms. Training has to be a factor. When is it ever okay to point a firearm at someone you don't intend to shoot? If the firearm is in someones hand it is their responsibility to keep the muzzle in a safe direction.

It got so bad at Camp Johnson that the sentry would just put his hands up and the guard officer would remove the pistol from his holster and place it in the next sentry's holster, So much for learning anything about safe firearm handling.

Played them like a dime store fiddle. 9M for a vacant lot that used to at least have property tax paid. Maybe they can build a school on it.
