LEE / STAFF By CLAUDIA PINTO Staff Writer Published: Friday, 10/28/05 Vanderbilt University Medical Center is one of five hospitals nationwide that are testing the effectiveness of an avian flu vaccine, which would be the only real defense against a potential pandemic that experts say could kill as many as 150 million people. The avian flu is a contagious disease among birds. But dozens of people in Southeast Asian countries have become sick and died from it, typically after tending infected poultry. The fear is that the virus will mutate, learning to spread from person to person, and grow quickly out of control unless a vaccine becomes available. "There is evidence, in at least one situation, where it has been passed from one person to another," said Dr. Kathryn Edwards, a Vanderbilt professor of pediatric infectious diseases. "There are changes occurring every day in the virus. The concern some of us have is that the regular flu will mix with the avian flu, allowing it to pass very effectively from human to human. "Developing an effective vaccine is our best defense.'' Vanderbilt will test the vaccine in adults 65 and older. It previously has been studied in 450 healthy people ages 18-64 in research funded by the National Institutes of Health, a federal agency that funds medical research. That study, which ended about a month ago, indicates that the vaccine is safe and appears to be effective. None of the participants had serious side effects. The avian flu vaccine works like most vaccines, said Dr. Tom Talbot, a Vanderbilt assistant professor of medicine. It contains a dead part of the virus, which tricks the body into believing it's being attacked by an active virus. The body then produces antibodies to fight the germs. The antibodies stay in the body and are able to kill the real virus if a person later becomes infected. "The shots won't make people sick," Talbot said. "It's not a live virus." The Vanderbilt study will focus on fine-tuning vaccine dosage to find out what amount produces the best outcome in the elderly. Participants will receive three shots over six months. They'll get either high doses of the dead virus, lower doses or a placebo vaccine, with none of the dead virus. To determine which doses produce the best outcomes, researchers will draw blood and look at antibody response. Vanderbilt is seeking 80 healthy people ages 65 and older to participate. They will begin screening volunteers for the study today and hope to begin administering the vaccine next week. "We really need help from our community to solve this problem," Edwards said. There have been 62 deaths from the 121 cases of avian flu in Southeast Asian countries since 2003, according to the World Health Organization. Countries that have experienced outbreaks of the avian flu, such as China, Japan and South Korea, have had to kill hundreds of millions of chickens and other birds to try to halt the illness. While Talbot believes the avian flu is a real threat, he said there's no reason to panic. "Right now, it's only in the bird population. It's not even in the bird population on this side of the world. The message has not gotten out to keep perspective." While other avian flu vaccines are being created, this is the only one being tested. The Vanderbilt researchers said the Food and Drug Administration could approve it within a year and even earlier if a pandemic does occur. But Talbot said there's no way to know when it could strike. "We use history as a guide. There have been three pandemics in the last 100 years. The last one was over 35 years ago. We know we are due for one. "We don't know if it will hit in six months or six years, but you want to have it available for when it does hit.'' Vanderbilt's research is being funded by a $10 million grant from the National Institutes of Health. In addition to Vanderbilt, UCLA Medical Center, University of Cincinnati Medical Center, University of Maryland Medical Center and University of Rochester Medical Center are studying the bird flu vaccine. Volunteers sought for avian flu vaccine study October 31, 2005 Vaccine would be first to protect against bird flu Nashville (Tenn.)–Vanderbilt University Medical Center is enrolling volunteers in a study to test a new vaccine that targets avian flu, the first such vaccine against the virus. The Vanderbilt trial, led by Kathryn Edwards, M.D., will test the new vaccine in nearly 100 individuals 65 years of age and over. It is the second phase of a national study led by the National Institute of Allergy and Infectious Diseases. The first phase of the avian flu vaccine trial, conducted at various other medical centers around the country, tested the vaccine in normal, healthy adults and determined it was safe to test in other populations. The development of a widespread epidemic of avian influenza, or “bird flu,” is of growing concern throughout the world. Human cases, with a high fatality rate, have been reported in Vietnam, Thailand and Cambodia. Public health experts fear that avian flu could lead to a worldwide pandemic if the virus becomes easily transmittable, leading to a public health crisis. Currently, most people are highly vulnerable to infection from avian influenza, as the current influenza vaccines do not protect against this new strain of the virus, making the development of new vaccines against avian flu a major priority. “We believe in some cases in Asia that the avian flu has been passed from human to human, and we’re concerned that the virus will evolve to spread easily among the human population and become a global pandemic,” said Edwards, professor of Pediatric Infectious Diseases and vice chair of clinical research in the Department of Pediatrics. “Developing an effective vaccine is our best defense. The elderly, in particular, could be at high risk for complications and death from infection, so it is very important to test the new vaccine in older adults.” Avian flu first emerged in Hong Kong in 1997 and has spread among poultry populations in some Asian countries. Humans have contracted the virus from sick animals, and evidence has pointed to a small number of cases in which human-to-human transmission is believed to have occurred. Identified as H5N1, the strain of the influenza virus has shown a high mortality rate–approximately 72 percent–with many fatalities among previously healthy young people. The vaccine researchers are testing was made by Sanofi Pasteur from an inactivated H5N1 avian flu virus isolated last year. “It’s similar to the concept of the regular flu vaccine that people get every year, but it’s targeted to this novel type of flu,” Edwards said. Because the study is beginning at the start of this year’s influenza season, all participants will be given this year’s licensed influenza vaccine if desired. Participants will then be given three doses of either the bird flu or placebo vaccine over the next six months and will be required to provide blood samples to test if protection against the virus was produced. I may be off on this but as I understand it there is a bit of a problem with createing a vacine for a pandemic version of the bird flu. Basicly its a virus that dosnt exist at this time. Flu viruses mutate very readily and the main concern is that it will mutate in a way that it would BECOME a real threat to humans, it it mutates to be a threat it would not be quite the same virus it is now and so a vacine to what it is now likely would not be effective and any attempt to mutate it in a lab so as to get a sample of what would need to be protected against would just be a guess at how it would mutate. So while its good to see that there is real concern about it and things being set in motion to try to combat it, I personaly dont have much faith in a vacine being able to be developed prior to a major outbreak.
I was watching PBS special series on RX for Survival.... last night.... There is a vaccine that they isolated out of the current H5N1.... problem is there isn't enough time to make enough.... even if everyone started to make it... if this hits in the next six months.... or even a year.... there won't be enough for the world.... enough for the US but not enough for the world.... a woman expert on the show said if we kept it for only ourselves " that would make the US the mosted hated nation in the world"...... (of course in some places we've already achieved that status)..... also.... as Monkeyman says.... this thing could mutate in that time and the vaccine would be compromised.... all in all its a "crapshoot".... best thing is to stay healthy and hope your immune system is strong enough for this one.... the Ultimate.... Survival of the Fittest....
OK, SO how does the WHO get together every Spring and "Predict" what 4 strains will mutate into our next "Flu" and then come up with a vaccine for the Flu SHot that we can get each Fall? Seems like they are pretty smart in doing this in such a small amount of time... lucky? but, Methinks that maybe they are in cahoots with the makers of that strain... and maybe the one's that throw us for a loop are the naturally occuring strains...
